General Principles of Haematopoetic Stem Cell Transplantation. The Role of the Clinical Laboratory in High Quality Grafts Harvesting

Abstract: The success of the haematopoetic stem cell transplantation (HSCT) depends on both the sensitivity of the malignant cells to chemotherapy and the quality of the graft. A high quality HSC graft that can ensure the recovery of the immune system and haematopoesis must be sterile and must provide at least 2.5 x 106 CD34+ positive cells /kg of recipient weight. The achievement of such a graft needs a team work between the HSCT group and performing laboratories. In the Center of Haematology and Bone Marrow Transplantation, Fundeni Clinical Institute, between March 2000 and December 2005, 47 grafts were collected with peripheral blood aphaeresis from 38 patients and 9 healthy donors. The following procedures were used: stem cell mobilization program, daily white blood cells (WBC) count, cytometry blood count of CD34+ stem cells, graft collecting, graft preparation for preservation and in vitro quality tests, cryopreservation in liquid nitrogen. In order to obtain 2.5x 106 CD34+ stem cells /kg recipient weight, 1 to 3 aphaeresis were required. The cell viability just after harvesting tested by direct means, was 94-99%. In vitro cell cultures showed 39 - 1074 x 104 colonies GM-CSF /kg weight/sample. Microbiological screening tests were negative for all samples. Before graft infusion, cell counting, viability tests, flow cytometry and stem cell cultures were repeated. 30 grafts were infused with the recovery of the haematopoesis in 7-15 days.
