Predictive factors for the discordant immunologic and virologic response at patients with HIV infection/AIDS under antiretroviral treatment

Abstract:

The goals of antiretroviral therapy are the reduction of plasma HIV - RNA levels, the immune reconstitution - increasing the number of CD4 cells and a favorable clinical outcome. On the basis of immunologic and virologic responses, patients were classified in four groups: complete response (I+V+), complete nonresponse (I-V-), immunologic response only (I+V+) and virologic response only (I-V+). The objective of this study is to evaluate the possible predictive factors of discordant immunologic and virologic responses the number of reverse transcriptase nucleoside inhibitors (RTNI) administered, of non-nucleoside reverse transcriptase inhibitors (NNRTI), of protease inhibitors (PI) and the duration of treatment in months. This study was performed at the Infectious Diseases Clinic in Tg. Mures. The studied group consisted of 144 HIV infected patients, aged 7-60 years, with antiretroviral treatment, CD4 cell count and HIV RNA plasma level were determined at the Molecular Biology Laboratory of the Infectious Diseases Clinic in Tg. Mures. The CD4 cell count was assessed by flow cytometry (Becton Dickinson), the virus load were measured with the use of the commercial quantitative PCR techniques (LCx HIV-RNA Quantitative –Abbott and COBAS AMPLICOR HIV -1 MONITOR Test, v1.5). Results: 38,19 % of patients had complete response, 40,27% had discordant response, the most frequent response (30,55%) being the only immunologic response. We have found a statistically significant difference between the average number of administered RTNI in group I-V+ and group I-V-. This difference was found both between the average of administered RTNI and PI in the group I+V- and the group I-V- and the duration of treatment between the discordant groups and the complete nonrespone group. Conclusions: the elevated number of administered RTNI and the increased duration of treatment in months seems to be a predictive factor for discordant response.
